A $25 late fee may be added
to registrations and payments received after this date. The fee
also applies to on-site lecture and field registrations.

PLEASE NOTE: There will NOT be a Tuesday field session. Certification will take place Wednesday and Thursday only.

• Please do not send in faxed or photocopied registration forms.
• No phone registrations accepted.
• Paying by credit card and need a receipt? Please include an e-mail address with your registration form and we will send you a receipt.
• If paying by company check, the cancelled check is the receipt.
• ETA accepts the following forms of payment: Company or personal check; Visa; Mastercard; American Express; Cash for field registrations only. Do not send cash via mail.
