Field Information
• Tuesday (for field registrants only), Wednesday & Thursday (all others), August 12-14, 2008
• 8 a.m. registration and 8:30 a.m. start

• PLEASE NOTE: Only those who have previously certified can attend the Tuesday field session. All others will attend the field sessions Wednesday and Thursday.
Payment Information
• Please do not send in faxed or photocopied registration forms.
• No phone registrations accepted.
• Paying by credit card and need a receipt? Please include an e-mail address with your registration form and we will send you a receipt.
• If paying by company check, the cancelled check is the receipt.
• ETA accepts the following forms of payment: Company or personal check; Visa; Mastercard; American Express; Cash for field registrations only. Do not send cash via mail.
